advantage of the opportunity to receive 10% off the amount you pay
toward your medical plan coverage simply visit
CBS & YOU Portal at
www.cbsandyou.com
and click on the "Health Assessment" spotlight on the homepage and
complete the questionnaire between October 1 and November 30, 2011.
The online Health Assessment is confidential and
no one
at CBS will ever see your individual responses or
results.
Keep in mind that even if you took the Health Assessment last year, you
will still need to complete it again this year, if you want to continue
to receive the 10% discount for 2012.
